
Cook pasta according to package directions in a large pot of salted boiling water. Reserve about 100ml of pasta water before draining.
While pasta cooks, heat butter or oil in a large pan over medium heat. Add sausage and cook until browned and cooked through, breaking it up with a spoon.
If using, add onion and garlic to the pan with the sausage and cook for 2-3 minutes until softened and fragrant.
Add the diced carrot and chopped asparagus to the pan. Cook, stirring occasionally, for 4-5 minutes until the vegetables are slightly softened but still have a bite.
Stir in the diced tomato, milk, and about half of the reserved pasta water. Bring to a gentle simmer.
Add the drained pasta to the pan with the sauce. Toss to coat everything evenly. Add more pasta water if needed to reach desired consistency.
Stir in the grated cheese until melted and creamy. Season with salt and pepper to taste.
